I understand that martial arts training CAN CAUSE INJURY.  I hereby WAIVE AND RELEASE SERENITY TAEKWONDO from liability for any injuries incurred.  I understand that by signing this waiver and release, I willingly agree to assume complete and full responsibility for any risk of injury that may arise as a result of martial arts training and activities.  On behalf of myself, my heirs, assigns and next of kin.  I waive all claims for damages, injuries or death sustained to me or my property while in attendance at Serenity Taekwondo.   I understand that the activities I will participate in at Serenity Taekwondo are inherently dangerous and may cause serious bodily injury, damage to personal property and/or death.  By signing this waiver, I assume any and all risk and responsibility and agree to waive any and all claims of personal injury, including severe bodily injury, damage to personal property and death relating to all activities associated with Serenity Taekwondo.  The team activities, but not limited to the following: receiving lessons (either private or group lessons) at the facility, participating in martial arts classes, using the facility and its equipment, practicing and engaging in martial arts activities such as sparring, Parent Night Out and any Serenity Taekwondo related activities on or off the school premises.  If I am injured from said activity, I agree not to hold Serenity Taekwondo or any of its affiliates responsible or liable even if the injuries were caused by negligence on my part, Serenity Taekwondo or its affiliates.

I do not have any physical limitations, medical ailments, physical or mental disabilities that would limit or prevent me from practicing in the above mentioned activities.  If required, I will obtain a medical examination and clearance.  Serenity Taekwondo will not be responsible or liable for any previous health conditions or injuries acquired prior to enrollment at Serenity Taekwondo that may be worsened as a result of activities at Serenity taekwondo.  This contract is non-transferable.

Registration Fee:  New students must pay the annual Registration Fee of $75. It will renew each year on January 1st.

Sparring and Equipment : Students are eligible to spar only after advancing from White Belt. Sparring is a requirement to be eligible for future belt tests. All required equipment is available for purchase at the school. A complete Sparring Gear set is available for $300.00 plus tax. Prices for individual / replacement equipment vary. You may purchase and use sparring gear from outside of Serenity Taekwondo as long as there are no other school's names on the equipment.

Testing : A belt testing fee of $75.00+ will be charged immediately following the student’s belt test.

Due Date : Tuition is due on the 1st of each month if enrolled in autopay. Students will not be able to check-in if tuition is not paid.

Recurring Billing: You must have a credit card on file to select recurring membership. Please let the front desk know at least 1 week prior to the due date if you would like to be removed from autopay.

I grant permission to Serenity Taekwondo to use my image (photograph and/or video) for use in Media publications on official Serenity Taekwondo page(s) and/or affiliates. 

I hereby waive any right to inspect or approve the finished photographs or electronic matter that may be used in conjunction with them now or in the future, whether that use is known to me or unknown, and I waive any right to royalties or other compensation arising from or related to the use of the image.

3 Students: $175

If you are a new registration after July 1st the fee will be discounted by 40%.  The full Annual Registration amount will be due January 1st of the following year.

1 Student: $145 per month for autopay or $155 for 1 month 

2 Students: $240 per month for autopay or $250 for 1 month

3 Students: $335 per month for autopay or $345 for 1 month

4 Students: $430 per month for autopay or $440 for 1 month

5 Students: $525 per month for autopay or $535 for 1 month

Private Lessons:
